PHARMACOLOGICAL PROPERTIES
Pharmacodynamic Properties

Mastivil Intramammary Suspension is effective in the treatment of clinical mastitis during lactation caused by bacteria, primarily Streptococcus sp., Staphylococcus sp., E. coli, and Klebsiella pneumoniae, which are sensitive to Penicillin, Streptomycin, and Neomycin.

Mastivil Intramammary Suspension is successfully used in the treatment of mastitis cases caused by susceptible pathogens. The combination of procaine salt of Penicillin G, a long-acting type of penicillin, with streptomycin has a synergistic effect. The bactericidal effect of procaine Penicillin G causes irreversible interaction with the transaminase enzymes of bacteria in the multiplication phase, preventing glycoproteins from attaching to the bacterial membrane structure, leading to disruption of the bacterial membrane structure and death of the bacteria.

When administered intramammary, streptomycin is absorbed to a limited extent. It exhibits bactericidal activity depending on its concentration in the environment. The presence of pus, blood, and tissue debris in the environment does not alter its effectiveness.

Neomycin, on the other hand, is not absorbed by tissues, providing long-lasting local antibacterial activity at the site of application, thereby reducing the pathogens in the environment. Neomycin exhibits bactericidal activity by inhibiting bacterial protein synthesis.

Prednisolone, a synthetic glucocorticoid, has an anti-inflammatory effect that alleviates symptoms of inflammation.

Pharmacokinetic Properties

Penicillin levels in milk at 12 and 24 hours after infusion of Mastivil Intramammary Suspension were 24.80 IU/ml and 3.40 IU/ml, respectively, while streptomycin levels were 13.45 and 1.55 μg/ml. For neomycin, these values are 13.45 and 2.17 μg/ml. The values obtained in milk quantity determinations 12 and 24 hours after application are above the MIC values of Staph. spp., Strep. spp., and E. coli, which are considered the most important causes of mastitis. Of these, the penicillin MIC value for Staph. aureus is 0.0125-0.12 μg/ml, for Strep. dysgalactiae it is 0.06-0.25 μg/ml, and for E. coli, the streptomycin MIC value is 2 μg/ml.

AREA OF USE/INDICATIONS

Mastivil Intramammary Suspension is a broad-spectrum antibacterial preparation developed for the treatment of clinical mastitis occurring in dairy cows during lactation. It is used in the treatment of acute and subacute clinical mastitis during lactation caused by penicillin-, streptomycin-, and neomycin-sensitive bacteria such as Streptococcus spp., Staphylococcus spp., E. coli, and Klebsiella pneumoniae, as well as for the relief of inflammation and pain.

USAGE AND DOSAGE
Through the intra-mammary route, a tube is inserted into the affected breast lobe. Before administering the medication, the affected breast lobe is drained. The nipple and hands are cleaned to ensure antisepsis. The cannula cap is then removed. The cannula of Mastivil Intramammary Suspension is carefully inserted through the nipple and the tube is emptied. The nipple is rubbed with a finger and gentle massages are performed on the breast lobe with the palm of the hand to ensure that the medication is distributed throughout the breast tissue. After the medication is administered, the nipple is cleaned again with an antiseptic wipe. Treatment should be continued for 3 days at 24-hour intervals. If necessary, intramammary drug applications should be supported with systemic antibiotic applications. Milking should be performed 6 hours after application.

SPECIAL CLINICAL INFORMATION AND SPECIAL WARNINGS FOR TARGET SPECIES

The nipple and hands are cleaned and antisepsised. Then, the cannula cap is removed. The cannula of Mastivil Intramammary Suspension is carefully inserted through the nipple to empty the tube. The nipple is coated with a finger, and the breast lobe is gently massaged with the palm to distribute the medication into the breast tissue. After administering the medication, the nipple is cleaned again with an antiseptic wipe.

Use during pregnancy and lactation: It can be used during pregnancy and lactation.

ADVERSE EFFECTS

The most important side effects of penicillins are acute anaphylaxis and collapse. In addition, hypersensitivity reactions (such as urticaria, fever, angioneurotic edema) may occur less severely but more frequently. If anaphylaxis occurs, epinephrine and/or oxygen and IV steroids should be administered. Urticaria and skin lesions may be seen in animals known to be sensitive to the active ingredients in the composition.

DRUG INTERACTIONS

No adverse interactions have been observed with any systemic parenteral administration that may be used to support disease treatment.

It should not be used in conjunction with bacteriostatic drugs.
